Program Description

Strength-Based Case Management (SBCM) program delivers case management and wraparound supports to adults (ages 18-59) with serious mental illnesses who may also have a co-occurring substance use disorder. SBCM identifies and engages clients including those who may be homeless, or are at serious risk of homelessness, may be indigent and who are unserved and are users of psychiatric inpatient and medical services.

Position Description

  • Provides case management for an assigned group of clients including coordinating and monitoring activities of the treatment team from a strength-based perspective.
  • Develops, evaluates, and implements treatment goals and plans in conjunction with the treatment team.
  • Provides individual supportive counseling and symptom management.
  • Performs psychosocial assessments, and Alcohol and Drug screenings.
  • Coordinates the full range of client’s needs. Provides on-call services, linkage and advocacy to ensure resources are accessed in a timely manner, providing community-based services in client’s residence as needed.
  • Complies with all charting and documentation requirements per contract.

Education/Experience Requirements

  • Master’s degree in a behavioral science and at least three years’ experience providing clinical services to individuals with mental illness; OR
  • Bachelor’s degree in a behavioral science and at least four years’ experience providing case management services to individuals with mental illness.
  • Bilingual in Spanish, Vietnamese, Tagalog or Arabic is preferred if the position serves that population.
  • Knowledgeable of ACT, Psychosocial Rehabilitation, CCISC, and cognitive/behavioral models treatment.
